Medicare Annual Wellness Visit

The Medicare Annual Wellness Visit (AWV) is a yearly, preventive visit covered by Medicare for most enrollees. These visits are stand-alone appointments, scheduled separately from problem-focused visits, and are completed with your primary care provider or a provider who works closely with your primary care team.

The goal of the Annual Wellness Visit is to take a proactive, whole-person approach to your health by identifying risk factors early, preventing future illness, and supporting long-term well-being.

What Happens During a Medicare Annual Wellness Visit

The Annual Wellness Visit is not a head-to-toe physical exam. Instead, it focuses on prevention and planning for your health needs. During or before your visit, you will complete a Health Risk Assessment that helps guide the discussion with your provider.

During the visit, you can expect to:

• Review your health history and family history

• Record basic measurements such as height, weight, and blood pressure

• Review all current medications

• Assess safety risks and functional ability

• Screen for cognitive concerns and depression

• Identify overdue screenings or immunizations

• Receive referrals if needed

• Develop a personalized Preventive Health Plan for the year ahead

Your primary care provider is informed of all findings, recommendations, and referrals to ensure continuity of care. Any follow-up visits are scheduled with your regular provider.
